PCV/ PSV Medical
PCV PSV Medical, We provide you with full DVLA D4 Medicals for PCV, PSV, Bus and Minibus Driving.
All PCV and Bus Drivers must undertake a compulsory medical before applying for a provisional category D licence. A renewal of the medical must be completed at the age 0f 45 years and then every 5 years to 65. After the age of 65, this then becomes an annual requirement.
Minibus Medicals
If you passed your driving from 1997 onwards, you must undertake a medical in order to drive a minibus (category D1). Same as a PCV.

- DVLA STANDARD D4 MEDICAL FORM. Please fill in the grey section on page 1 (left hand side) ‘Your details’ and page 8 (consent and declaration) alongwith your name and date of birth at the foot of all other pages on the D4.
